Facility name and address, the dock door number if there's more than one, the equipment type if known (hydraulic, mechanical, or air-powered leveler; dock plate; dock door), what the equipment is doing or not doing, how long it's been happening, and whether the bay is currently usable. Photos of the pit, lip, or damaged area help a provider evaluate the job before they arrive.
Urgent versus routine requests
